We provide a personalized Baseline Visit to put you in control of your health from the start. Meet with your Care Navigator and a clinician (in available states) to get a full picture of your health and better understand your benefits. When you complete your Baseline Visit within the first 120 days of the plan start date, you continue to get $0 out-of-pocket costs for in-network care.
